All Members with Referral Service must review this profile for accuracy every four months. Please read this guide [pdf] for more information on the Referral Service.

Select your availability in the form below, and include only the treatment and assessment areas you are comfortable receiving referrals for. Scroll all the way to the bottom to indicate your referral address(es) and phone number(s). The address you provided during membership signup will not be available to the public, unless you also include it as a referrral address on this page.

To ensure that members of the public can also search for you by gender, please visit the "Name and Title" tab and make sure that your gender is selected. If no gender is indicated in our system, you will not come up in referral searches that include gender.

The "Website" field is where you should type the URL of you external website, if you have one.

Please make sure to enable the referral service (scroll to the bottom of this page), otherwise you will not appear in the referral search. Make sure to click on "Save" at the bottom of the page if you made any changes to your referral settings, otherwise your changes will not be saved.
